Retrograde endoscopic cholangiopancreatography in the diagnosis of cholestasis.

Analisámos a nossa experiência com a CPRE em 238 casos consecutivos com icterícia colestática. A CPRE foi diagnóstica em 193 (81%). 159 tinham colestase extrahepática (99 benigna; 60 maligna) e 34 colestase intrahepática. Nos últimos 50 exames, as nossas percentagens de exames diagnósticos e de visualização das vias biliares ultrapassaram os 90%. Em 34 doentes, a endoscopia e pancreatografia forneceram elementos decisivos de diagnóstico. Não se registaram complicações atribuíveis à técnica. O papel da CPRE na abordagem da colestase é avaliado com base na nossa experiência, e valorizado em função de 3 parâmetros: máxima informação diagnóstica, risco mínimo e relação custos/benefícios favorável. Conclui-se pela necessidade de incluir a CPRE no conjunto de meios diagnósticos da colestase nos centros hospitalares de gastrenterologia do nosso país.
